Tallahatchie General Hospital Administrator Jim Blackwood said a grant award of $125,790 will help to provide the facility better real-time insight into the health of patients enrolled in the medical facility’s Chronic Care Management program.
On Monday, the office of Gov. Tate Reeves announced 167 Mississippi Rural Health Transformation Program awards totaling more than $104 million to medical facilities around the state, including the TGH monies given under the Rural Provider Technology Grant Program.
Blackwood said the local award was in response to an application TGH submitted seeking to provide existing chronic care management patients with remote patient monitoring equipment that would allow the TGH care team to receive “consistent, real-time measurements of conditions that the patient seeks to manage.”
Examples of that equipment, he explained, would be cellular enabled blood pressure cuffs, glucometers, pulse oximeters and more.

“This funding represents an important investment in the health of our community,” Blackwood said. “By partnering with Lynk Health to implement remote patient monitoring, Tallahatchie General Hospital will be able to provide more proactive support to the nearly 400 patients enrolled in our Chronic Care Management program. The technology will help our care team identify concerns earlier, improve patient engagement, and ultimately achieve better outcomes for those living with chronic conditions.”
Reeves said this week’s awards represent the first of three grant opportunities in the first year of a five-year plan to modernize, strengthen and transform health care in rural communities in the state.
